Sub-Aquatic Sports & Service -- Learning Agreement

 

Welcome to your scuba course; it's going to be a great time! Diving is a very diverse hobby, allowing you to explore sunken shipwrecks in the Great Lakes, admire the beauty of our oceans, swim with dolphins, traverse caves and caverns and so much more. This course is designed to be fun, but, like any form of learning, you'll need to invest time and effort in studying and showing up to each session. Although diving is exciting, please be aware that there are risks involved with the sport. You need to be comfortable in the water as well as in good health. On the first session, a medical history form must be completed. The instructor may require a physician examination if they feel it necessary (i.e. asthmatic students). Minors must have a parent or guardian sign the medical form and waiver on the first session.

 

Students Responsibilities:

#1 Complete all sessions of the course. Because each session covers essential information, you will be required to make-up any missed sessions. The lectures may be completed via DVD purchased at Sub-Aquatic Sports for a minimal fee. Missed pool sessions are rescheduled directly with your instructor at a time convenient for both of you. An additional fee of $40 for a private pool session may apply and paid directly to the instructor. If you miss more than two sessions, you will need to retake the class (at an additional cost) to become certified.

#2 Provide the adequate equipment and material for the course. For the first session of class, please bring a writing pad to takes notes on, a swimsuit and a towel. Some people also like to bring a lock for the lockers to secure personal items. By the second session, you will need snorkeling equipment (mask, fins, snorkel and weight belt); we'll discuss how to purchases this gear wisely during the first session. We provide scuba equipment for the pool sessions (tanks, regulators, buoyancy compensator vests) but because snorkeling equipment is of a personal nature, it is required. Our store has a large selection of equipment and you will receive a student discount. Scuba equipment is not provided for Open Water Training; if you need to rent gear, the cost is $60/weekend. Finally, the NAUI textbook/dive table combo ($45 and up) is required: please read pg. 1-20 prior to the class starting.

#3 Complete Open Water Training dives within 6 months of taking the classroom/pool portion of the class. Training dives are the last step of your scuba certification. There are several training dive options, which your instructor will discuss during the class. Four scuba and one snorkel dive must be successfully completed. You must complete these dives within 6 months or retake the entire classroom/pool portion of the course at an additional cost.

#4 Pick up your certification card at the store . The good news is that your NAUI certification is good for life; you never need to renew it. The bad news is that the card is processed 2-3 weeks after the open water training. It will not be mailed to you unless strongly requested. Because we have lost numerous mailed cards (probably due to the fact they feel like credit cards), we cannot guarantee the mailing. If you that it is mail and it is loss, you will be responsible for paying for a new card, $30.

#5 Your non-refundable $50 deposit reserves your place in the course . The balance of the course fee is due 1 month prior to the first meeting. The balance of the class is refunded if given a 14 day notice.

 

I understand that in taking this course, I am incurring an obligation for attendance, skill performance and financial responsibility. I understand that diving is physically exerting sport and that I am comfortable in the water. I am willing to accept the risk and responsible for my own actions. I understand that the instructor must make the final judgment as to my competency to be a safe diver and be awarded certification.
